Create a maintenance request from checklist actions

As a maintenance worker, I want to automatically create a maintenance request based on my response to the work order checklist.

Add new action type "Request" to checklist actions, with setup of:

• Checklist action ID

• Active (Yes/No)

• Checklist action type (Work order, Request, Alert)

• Request type

• Service level

• Request description with placeholders, ex. [asset], [functional location], [checklist varible] etc.
  (It would also be great if we could setup work order description with placeholders on related work order checklist action)

Explanation – We utilize checklists quite a bit for some of the work groups. With checklist actions, we would like the functionality that if a defined parameter is met or exceeded that we could create a work order (already a function), create a Maintenance request (which could either be made into a WO or Rejected), or have an email sent to a defined email list such as in the functionality similar to Custom Alerts. Either the maintenance request or email alert would notify the supervision and they would decide if action may be needed or not.
